How do I fill in the PIP form?
The form is called How your disability affects you. Answer each task with one real example, how many days it happens, and any help or aid. The decision maker is asking if you can do it safely, to an acceptable standard, repeatedly and in a reasonable time, on more than half of days.
- Send it back within one month of the date on the letter. Ask for more time before that month ends if you need it.
- Do not write only the diagnosis. Two people with the same condition can score differently.
A usable answer
Say what you were trying to do, what stopped you, how long it took, and what happened next. Name the person who helped and what they did.
Repeat that shape for every task that is hard. Skip a task only if you genuinely have no difficulty with it.
Good days and bad days
Say how many days in a week are bad. If it is most days, say most days. If it is some days, say some days. The test is more than half.
A diary kept on the real days is stronger than a form filled in from memory on one afternoon.
What to send with it
Appointment letters, a prescription list, a care plan, and a short note from the person who helps. Send copies.
Keep a copy of the form. If you later ask for a reconsideration, you will need to know what you said.
